REGISTRATION INFORMATION
Pre-registration is required for all classes. Registration is on a first come, first served basis. There is a minimum and maximum number of participants for each activity. The day a class starts, online registration will be closed.
Payments: Payment must be made at time of registration.
Cancellations: If enrollment is insufficient, the activity will be canceled and all money refunded.
Refunds: Participants may cancel within 48 hours (2 workdays) before the start of class/rental and receive a refund less a $10 withdrawal fee. If requests for withdrawal or cancels are made after the start of a class or rental, the fee will be forfeited with a maximum of $15 for illness/injury. Refunds are made at the discretion of the Div. Superintendent/Director. Stop payment of a check nor failure to show up for a class doesn't constitute withdrawal from a program.
Discrimination: The Carrollton Parks, Recreation, and Cultural Arts Department does not discriminate on the basis of race, color, sex, national origin, or disability in any of it's programs, activities, or employment.

Policies: CPRCAD is not responsible for any injury incurred while participating in any of its activities or facility use. Enrollment age for each class is noted in the description of a class. Participants are not permitted to bring children to a class.
EZ-REG is our updated program registration process.
There are now four ways to register.

In order to use EZ-REG Web, you will need personal access codes. These codes include a Family Account PIN and individual Client Codes for each house-hold member. You will need these account codes each time you plan to register. If you are a new client, complete a Registration Form and return so we can create an account for you. This form is available at the office on 118 S. White Street, may be downloaded at www.cprcad.org or sent upon request by calling (770)832-1161.
Current participants may call (770) 832-1161 or drop by 118 S. White Street to get their personal access codes. Even if you do not use EZ-REG Web to register and pay for classes, you can view your household's account balances, current registrations, and past transactions for all clients in that account. Our electronic system is secure and allows you to register 24 hours a day from your home or office.
First time participants must fill out a Registration Form. You will find this in our catalog, at our web site or call our offices and we will mail or fax you one. Payments may be made with cash, personal check, money order, VISA/MasterCard. Make checks or money orders payable to CPRCAD.
EZ-REG WEB Our site address is www.cprcad.org. You will be able to register for classes, check class availability, view your account information, or make payments 24 hours a day. Some classes are not available for registration using this system. Please read program descriptions carefully. You must enter payment to confirm registration.
EZ-REG WALK-IN Visit our offices at 118 S. White Street, Carrollton, Georgia. Office hours are Monday-Friday, 8:30 a.m. to 5 p.m. Business Phone is (770) 832-1161.
EZ-REG MAIL-IN Fill out and sign a registration form. Mail it with your payment to: c.p.r.c.a.d., P.O. box 532, Carrollton, GA 30112.
EZ-FAX Fill out and sign a registration form. Fax to (770) 834-3652. Then call us with your payment information.
Registration must be done no later than one day before a class starts in order to use this form of registration.
